TF3 Consortium - Solutions

TF3 understands that the solution starts and ends with a deep understanding and respect for the patient and their needs. We deliver a technology enabled and not a technology-driven service that will put patient interests at the centre of everything we do.

TF3 takes a technology agnostic approach to patient care. It does this by providing an infrastructure which supports multiple devices and by providing a choice of telehealthcare solutions from the outset, bringing the following benefits:

Telehealthcare is an umbrella term for:

Telehealth
Telehealth is the consistent, reliable and accurate remote monitoring of a patients vital signs and the delivery of medical care at a distance, enabling day to day individual care according to need and promotion of self management
Telecare
the continuous, automatic and remote monitoring of real-time emergencies and lifestyle changes over time to manage the risks associated with independent living.

Telehealth

We find patients will call us if they have had an alert on the telehealth system and feel an exacerbation developing ... but to tell us not to 'panic' as they have started their oxygen treatment to counter the symptoms! This type of expert patient is exactly what we are striving to develop! Lead Respiratory Nurse

TF3's telehealth technology focuses on effective remote monitoring of a patient's vital signs through the use of easy to use equipment that professionals can customise to their needs and to each patient, enabling day to day individual care according to need.

Our telehealth software packages allow clinicians to automatically prioritise patient care based on individual need, defined and owned by the clinician. Held in a secure data storage facility, it is easy to access for remote users and alerts clinicians when a patients condition changes even when they are out on visits. It also offers interoperability and integration with existing healthcare systems.

Telehealth promotes proactive case management, reduces costs, empowers patients and promotes higher professional morale. It increases patient self-management allowing patients and family members to experience a reduction in anxiety and stress and patients learn to recognise their own symptoms and when they may need intervention, increasing independence and confidence.

It offers more support for evidence based decision making, focusing care on patients who need the most support, ensuring the most effective use of limited case management resources.

Since telecare, I have started to go out again and spend time with ALL my family, and I know Mum will be ok. The equipment really gives me peace of mind. Daughter and mother

Telecare offers a way of helping to minimise potential risks in the home and help people remain independent in their own homes for longer. Unobtrusive sensors are placed around the home which automatically raise an alert if the detect a possible problem, such as smoke, gas, flood, fire, or a fall. The sensors are wireless and a flexible solution can be created to suit the needs of the individual. As a person's needs change over time, the telecare solution can be adapted to ensure they are always receiving the most appropriate level of support.

Telecare sensors are remotely monitored on a 24 hour basis and when triggered they will raise a local audible alarm to warn the user. They also automatically raise an alert with a monitoring centre, carer, or family member who can respond appropriately.

Technology can allow a carer to have a good night's sleep, safe in the knowledge that should their loved walk away from their the house at night, cause the bath to overflow or even leave their bed for a prolonged period of time, they will be alerted quickly and appropriately. Telecare enables carers to manage difficult situations in an effective and reasonable way. Such technology can also make spending a short time away from the house a possibility for the carer, which can make an enormous difference to the quality of everyday life.
